Jarvis with consent forms for salons and suites

Suites and salons collect health questions and aftercare on paper that never makes it back to the booth. A link in the booking text fixes that. Clients sign before they sit down; you keep an encrypted PDF if anything is questioned later.

Permanent makeup, microblading, and lash extensions each have their own template because aftercare and patch-test questions differ. Waxing, facials, and color services have lighter consent forms in the same gallery.

Use kiosk mode on a shared iPad at the front, or a QR on the mirror. Take a deposit on the same form if you connect Stripe or Square on a paid plan.

Not a medical product. Not legal advice. Edit aftercare to match what you actually tell clients.

Questions

PMU vs microblading — which form?

Use the permanent makeup consent for machine PMU (brows, liner, lips). Use the microblading template when the service is microblading specifically. Edit the treatment description either way.

Do fills need a new signature?

Many lash studios collect a full consent for the first set and a shorter fill acknowledgment later. Duplicate the template and trim the fill version in the builder.

Can the form go out with the booking link?

Yes. Paste the client link into your booking confirmation or reminder text so the form is done before they arrive.
