Permission slips for schools and groups
The slip is in the email. You are not collecting carbon copies at the classroom door.

Field trips fail when three slips are still in backpacks. A link in the class email, with a QR printed for pickup, gets a parent signature on any phone before the bus leaves.
Use parental consent for ongoing programs and clubs. Use the field-trip template when you have a destination and date. Youth sports and overnight travel have their own forms (sports waiver, minor travel consent).
If your district publishes required language, start from that sheet — Jarvis can draft from a photo — then keep the signed digital record instead of a crate of carbon copies.
Not legal advice. Follow district policy when it conflicts with a template.

Questions
Parental consent vs field-trip permission?
Parental consent covers ongoing participation. Field-trip permission adds destination, date, and trip-specific risks. Use both when a club also travels.
How do teachers see who is missing?
Check Submissions for that form the morning of the trip. Resend the link to outstanding parents or print a QR at pickup.
What about overnight or out-of-state trips?
Add the minor travel consent when a child travels without both custodial parents. Some trips also need a sports or activity waiver.
