Youth, camps & schools
Free parental consent form
Parents sign from home. You are not chasing paper slips on the morning of the trip.
Youth programs need a parent or guardian on the record: who the child is, who is consenting, and what they are consenting to. A stack of crumpled permission slips is how names get missed.
Use this general parental consent for ongoing programs and clubs. Switch to field-trip permission when there is a destination and date, summer-camp consent for session packets, or sports waiver for athletics. Camps and schools hubs show how those pieces fit together.
Share one link in the signup email. Each parent signs on any device. You see who is still outstanding in Submissions before opening day or the bus leaves.
This is not legal advice. Dual-parent households can add a second signature field or submit twice.

Questions
Can two parents sign the same form?
Add a second signature field in the builder if both must sign. Or send the link twice and keep both submissions.
What if a parent does not have email?
Print the QR at pickup. They sign on their phone in the parking lot. You still get the encrypted record.
When should I use field-trip permission instead?
When the outing has a specific destination and date. Keep parental consent for season-long enrollment and add field-trip permission per trip.
Do camps need more than this form?
Usually yes — photo release and sometimes activity waivers. Start from the camps hub packet list so opening day is not three separate paper chases.
