Forms for camps and youth programs
Parents sign from the kitchen table. Opening day is not a paper chase.

A camp packet usually needs parental consent, a photo release for the recap reel, and sometimes a field-trip or swim waiver. Start from the summer-camp and parental templates, then add photo release — or ask Jarvis to merge the questions you already ask on paper.
Send one link in the registration email. Parents sign on any phone. Submissions shows who is still outstanding so you are not chasing backpacks on opening morning.
Print PDFs only if your board still wants a binder. The encrypted record is the source of truth; paper is optional.
Templates are a starting point, not legal advice. Youth programs should have an attorney review the packet.

Questions
What belongs in a typical camp packet?
Parental consent (who can participate), photo release (if you post kids), and any activity-specific waiver (swim, climbing, off-site trips). Field-trip permission covers single-day destinations.
How do I see who has not signed?
Open Submissions for that form. Filter or scan for missing names before drop-off. Resend the same link to parents who are still outstanding.
Can two guardians sign?
Add a second signature field, or send the link twice and keep both submissions. For travel letters, both custodial parents often need to sign.
